REPLACEMENT POLICY

$5.00 Replacement Fee!

If by chance you happen to miss-place or loose your certification card, we will charge you a replacement fee of $5.00.  You will need to contact us by phone or e-mail.  If you happen to live within the Weber, Davis or Salt Lake City County areas, then we'll be more than happy to briefly meet with you in person in order to re-issue you a new certification card in exchange for a $5.00 fee. 

If you happen to live out side of Weber, Davis or Salt Lake City County, then you will need to mail us a check or cash.  Your mailing also needs to include a formal letter that needs to include your (business name if applicable) Your First & Last name, address, city, zip & phone number and a brief explanation for why you need a replacement card.   This will allow us to look up our records and make sure that we've recently trained you. 

If you send us a "check", then we'll have to make sure it clears with our bank first.  Unfortunately we've had too many clients write us a check that doesn't clear.   But if the check you send us clears, then we will be in touch with you about making arrangements in order to get you your replacement card. 

We are NOT willing to travel to Cache Valley, Mid or Southern Utah just to re-issue a single certification card...sorry.
